MODULE 1 Early Pregnancy Ultrasound From First Principles to Clinical Confidence |
Every early pregnancy scan has the potential to change a patient's life — for better or worse. Ectopic pregnancy is the leading cause of maternal death in the first trimester — and the majority of those deaths are preventable with a systematic, well-trained approach to ultrasound. In this module, you can build a framework for early pregnancy scanning that is systematic, reproducible, and clinically defensible — one that you can use every day for the rest of your career. |
20+ Clinical concepts |
9 Ectopic sites covered |
4 Twin types explained |
3 GTD forms clarified |
What You Will Learn in This Module |
The module starts with something that most courses skip — probe technique. You will learn exactly how orientation works for both transabdominal and transvaginal scans, how to navigate to the ovaries systematically, and how hand position affects on what you see. Residents who have completed a postgraduate degree still struggle with TVS orientation. After this session, you will not be one of them.
From there, the module builds through every component of a complete first-trimester assessment — from identifying a 4-week gestational sac to assigning chorionicity in a twin pregnancy to recognising the signs of gestational trophoblastic disease. Every concept is explained in simple language, supported by real-world examples, and linked to clinical decision-making.
→ |
How to use both transabdominal and transvaginal probes with correct orientation — including the thumb rule that solves confusion permanently |
→ |
What a real gestational sac looks like — and how to spot a pseudosac that signals ectopic pregnancy |
→ |
The double decidual sac sign, intradecidual sign, and double bleb sign explained in plain terms |
→ |
How to date a pregnancy correctly — the MSD formula, the CRL formula, and when to use each |
→ |
The 20–25–7 rule for diagnosing a failed pregnancy — and the crucial difference between failed and uncertain viability |
→ |
What a pregnancy of unknown location (PUL) actually means clinically — and the three-step surveillance plan that keeps patients safe |
→ |
How to diagnose an ectopic pregnancy before it ruptures — the bagel sign, the blob sign, and the ring of fire (and why ring of fire alone is never diagnostic) |
→ |
All nine sites of ectopic implantation — including interstitial, scar, cervical, ovarian, and heterotopic — and what makes each dangerous |
→ |
How beta-hCG trends influence diagnosis — when a rising hCG can still mean ectopic, and when a falling hCG may also |
→ |
Corpus luteum versus ectopic: the beak sign distinction that separates a common finding from a clinical emergency |
→ |
How to assign chorionicity and amnionicity in twin pregnancies — the lambda sign, T-sign, and why this must be done at the first scan |
→ |
Gestational trophoblastic disease: complete versus partial mole, theca lutein cysts, and the AV shunting rule that prevents catastrophic haemorrhage |
What You Will Walk Away With |
✔ A six-question framework — P-L-S-V-N-D — that structures every first-trimester scan from start to end |
✔ The ability to diagnose an ectopic pregnancy confidently, including rare sites that are commonly missed |
✔ A clear understanding of the failed pregnancy thresholds and what 'uncertain viability' involves in practice |
✔ The tools to assign chorionicity in twins at the first scan — and the knowledge of why that assignment cannot be changed later |
✔ Reporting language you can use immediately — structured, precise, and clinically appropriate |
✔ Confidence to handle the high-pressure situations that early pregnancy scanning regularly produces |
